Patient Clarification Text on Personal Data Protection (KVKK & GDPR)

1. Data Controller

In accordance with the Turkish Law on the Protection of Personal Data No. 6698 ("KVKK") and international privacy standards (GDPR), your personal data is processed by Lokman Hekim Ağız ve Diş Sağlığı Hizmetleri Limited Şirketi (operating Özel Lokman Hekim Ağız ve Diş Sağlığı Polikliniği) as the data controller.

2. Collected Data & Purposes of Processing

Your identity, contact, appointment, and dental health records are collected and processed for:

• Conducting oral examination, diagnosis, treatment planning, and medical care,
• Managing appointment requests, scheduling, and patient notifications,
• Fulfilling legal obligations mandated by the Ministry of Health and healthcare legislation,
• Maintaining patient files, medical history, and billing administration.

3. Data Sharing & Confidentiality

Your medical and personal data is treated with strict medical confidentiality and is only shared with authorized statutory bodies (such as the Turkish Ministry of Health) as required by healthcare regulations. We do not sell or transfer patient data to unauthorized commercial third parties.

4. Your Rights

Under Article 11 of the KVKK, you have the right to learn whether your data is processed, request access to your records, demand correction of inaccurate information, and request data erasure in accordance with statutory retention laws.
